What Is a Family-Centered Medical Home?
Children’s Specialized Hospital is your Medical Home, or your healthcare
home base. We are your central resource for all of your child’s
healthcare needs. Our family-centered medical home treats children birth
through 21-years-old, providing comprehensive and specialized primary
care coordinating your child’s care from start to finish. Together,
our healthcare team will provide a simpler approach to your child’s
healthcare. Through this partnership, you will receive the best resources
and information related to:
- Pediatric specialists providing coordinated care
- Information on health conditions and the latest treatments to better serve your child
- Home care and improved processes
- Equipment and quality service
- Support and respite services to protect and better your health
- Other key local resources specific to your area
Access and Communication
Our practice is available whenever you need us. Please discuss any special communication or access needs that you and your family have.
If you are referred to a specialist, with your permission, we will share information with them and you in a timely manner.
As Your Medical Home, we will:
- Take care of your child when he or she is sick
- Help you plan your child’s care and set goals for his or her care
- Discuss any testing or treatment that your child needs
- Coordinate care with you and other care providers
- Partner with you to help you live well and stay well
How You Can Prepare for Appointments:
- Plan to share general information about how your child is doing
- Write down and bring questions and concerns to appointments
- Prepare a list of your child’s providers and their contact information
- Provide a complete medical history and any current medications
Our Approach:
Our practice strives to address the developmental and medical needs of
children, in addition to behavioral health services. This is done through
highly coordinated and effective family-centered medical care involving
social workers, patient care coordinators, behavioral health therapists,
doctors and nurses.
